Liver-directed therapies will have multiple benefits for patients, Cusnir explains. Utilizing Y-90 resin in patients with advanced CRC to target the liver, the main sight of disease in most patients, may allow patients to receive intervals of systemic treatment, as well as prolong life, Cusnir says.

Y-90 resin should have an earlier role in the treatment of patients with CRC in the maintenance setting to achieve the optimal response, Cusnir continues. There may also be a spot for Y-90 resin in the chemorefractory setting, as there are many heavily pretreated patients that are seen for clinical trials that have never been treated with Y-90 resin, Cusnir adds. Ultimately, disease that is localized in the liver is where Y-90 resin will excel, Cusnir concludes.

Chemotherapy is typically effective in all lines of CRC, Cusnir says. Chemotherapy has been used in a variety of settings, from adjuvant therapy to curative intent, Cusnir adds. A role for chemotherapy can also be found in the advanced setting for patients with stage IV disease, Cusnir explains.

One key factor when evaluating a patient for the use of chemotherapy is the risk of adverse effects, Cusnir continues. Clinicians must consider how to limit chemotherapy-related toxicities for patients without compromising survival and quality of life, Cusnir concluded.

Ramucirumab, according to Cusnir, is one of the more interesting agents in gastric cancer due to an antiangiogenic agent being used in the first- and second-line settings. Data looking at combinations with ramucirumab are emerging and appear promising, according to Cusnir.

The ramucirumab data can be considered in different patient populations, though the agent appears to have the most activity in patients with borderline poor-performance status, concludes Cusnir.

Regorafenib is still in the early stages of development in gastric cancer, says Cusnir. Although regorafenib has shown activity in this space, it is also one of the more toxic agents. As such, it is unknown what its optimal role will be in the treatment paradigm.

In colorectal cancer, the phase II ReDOS trial demonstrated that a dose-escalation strategy could be used with regorafenib to reduce associated adverse events and maintain efficacy; however, it is unknown whether the same holds true in gastric cancer. In the absence of those data, the field is looking toward larger confirmatory trials to establish whether regorafenib has a role in subsequent lines of gastric cancer treatment, concludes Cusnir. &#8239;
