Hospitals and academic medical centers are paying more attention to the well-being of their trainees as burnout rates remain high in residency and fellowship training. Although graduate medical education (GME) at many hospitals includes provisions to ensure that trainees remain physically, emotionally, and mentally healthy, just a few of the residents and fellows make use of these resources. Discounted gym or group training memberships and joint services through departments of psychiatry for counseling, stress management, and mental health services often go unused as trainees struggle to find enough hours in the day to care for their patients and care for themselves.

Residency and fellowship are synonymous with embracing new norms while surrounded by uncertainty, whether than means treating patients during a pandemic, applying for a match, or comforting the family member of a dying patient. That said, you can only perform your best if you first take care of yourself. What follows are a few critical things that we wish we had implemented a lot sooner.

Only coffee for breakfast and then hoping for chiefs to arrange the best lunch during noon conference is the norm for trainees—and the “best lunch” is often pizza from a local pizzeria, cold wraps from the closest deli, 

or some poor facsimile of Indian, Chinese, Thai, or Mexican food. This assumes that the GME is not working on a limited budget and that the program chiefs have the funds allotted to order food. Otherwise, lunch may be just crackers, peanut butter, or applesauce from the ICU fridge (while we justify to ourselves 

that we are not stealing). By the time you’re home from your clinic, short call, or long call, preparing dinner is the last thing on our mind. Relying on takeout or quick microwavable meals is the fastest, cheapest option.

Plus, you’ve barely had any water all day long. Even mild dehydration can cause fatigue, and you’ve got more than enough reasons to be fatigued without adding another. The National Academies of Sciences, Engineering, and Medicine recommends daily water intake of approximately 15.5 cups (3.7 liters) for men and 11.5 cups (2.7 liters) for women, depending on activity level.

 It probably isn’t necessary to stress the importance of keeping hydrated to an audience of physicians, but it’s easy to forget about water when you’re grinding through a busy day. Plus, proper hydration can help you maintain focus.

A few things that Rohit implemented shortly after being exhausted halfway through his first year of 

fellowship included meal preparation and carrying a bottle of water at all times. Every weekend, he would spend a few hours preparing 5 to 6 balanced meals that usually included some carbohydrates and a source of protein, along with some raw seeds or nuts. He also became a big proponent of smoothies because you can pack a lot of good nutrients in a single smoothie. Rohit also made sure to carry his 1-liter reusable water bottle everywhere. 

Meal prep and remembering to carry a water bottle sometimes got tedious. But as Rohit saw the benefit of being prepared for each week, it soon became a habit.

Take a minute, close your eyes, and imagine what it would be like if you had started stretching or working out or running for just 30 to 45 minutes a few times a week during your college days? We all have heard the rhetoric around saving money early because of the magical effects of “compounding.” It is no different when it comes to fitness. The earlier you start, the better it is for you. But it is never too late to start. Five years from now, you do not want to ask yourself, “What if I started 5 years ago?”

It’s easy to lapse into bad habits and let your fitness routine fall by the wayside. Residents are at increased risk for gaining weight due to changes in environment, resources, stress level, and nonadherence to healthpromoting practices. Findings from a study conducted by Leventer-Roberts and colleagues showed that residents were more likely to be overweight (BMI ≥ 25) at the beginning of PGY-3 than at the beginning of postgraduate year (PGY)-1 (49% vs 30%; OR, 2.26; 95% CI, 1.19-4.28).

This should not come as a surprise to most of us. This is not about body shaming, nor are we recommending that you drop everything and start training for your first triathlon. But finding some time to invest in your own personal body is essential.

Little things can make a big difference. While in the hospital, Rahul was more conscious of using the staircase instead of the elevators. What worked best for him outside of work was signing up for racquetball and attending a group training session once a week with other residents so that they could all hold each other accountable.

Data from a meta-analysis of more than 17,500 residents showed that up to 43% were positive for depression or depressive symptoms.

 Furthermore, findings from a study of 654 medical interns showed that 42.5% were positive for depression. Of those, just 22.7% sought help during the intern year. The most-often cited barriers to seeking treatment included lack of time (91.5%), preference to manage problems on their own (75.1%), lack of convenient access (61.8%), and concerns about confidentiality (57.3%).

 Though the majority of these physicians showed no clinical signs of impairment, these depressive symptoms often lead to burnout. The data are yet to mature, but it is likely that these numbers will be higher in due to the coronavirus disease 2019 pandemic.

During his training, Rohit was fortunate to have good social support at work and outside of work. Implementing 5 to 10 minutes of daily mediation in his routine has also played a pivotal role. However, there has to be a systematic shift throughout GME, and physicians in training should have more support and structured time to address mental health individually and in a safe environment.

Looking back, each and every single day was long by itself, but 6 years in total of training flew by. With the chance to do it again, we would certainly be more mindful of our mental, physical, and emotional health. Neglecting your own needs could come at a very steep cost in the future.

There is no need to put “Join a gym” at the top of your New Year’s resolution list—make it a goal for tomorrow. Or, even better, today. Make eating healthy part of your daily routine so it becomes a habit. Take charge of yourself so you can live long and lead a better life, which can enhance not only your academic resumé, but also your life resumé.

Make Time for Your Own Health

 Your pager goes off just after midnight, with the dreaded words, “CONCERN FOR ACUTE LEUK. CALL TO DISCUSS. ED” displayed on the green fluorescent screen.

Still reeling from setting up emergency overnight apheresis for a patient with leukemia, you get 3 new consults the next morning, with requests ranging from interpreting a preoperative abnormal partial thromboplastin time test (surgery) to management of sickle cell crisis in an antepartum patient (OB/GYN) to evaluation of hemophagocytic lymphohistiocytosis in a patient with multiorgan failure (ICU). Taking a deep breath before calling the teams back, you suddenly remember that oncology continuity clinic is tomorrow morning and you still need to chart review the new patients…

Lots of fellows have drawn parallels between the hectic first year of fellowship and the intern year of residency—the inpatient services are fast paced, there’s plenty of assigned calls, and the volume of day-to-day learning feels like drinking from a firehose. In the eyes of other primary teams, the new fellow is suddenly expected to be the expert of all things hem-onc even if he was just a senior resident a few weeks ago. 

Like every stage of medical training, however, the days will get easier. Here are 5 tips that I found to be helpful as I look back on my first year:

This old adage still rings true in fellowship. Attending physicians are there to help you learn, and because fellowship is the terminal stage of medical training, it is even more important to ensure that you fully understand the rationale of clinical decision-making. In fellowship, attendings are less concerned with Christopher Su, MD, MPH double-checking your work (that’s what residency is for—now you’re a licensed physician). Instead, they want to help you develop into one of their colleagues. Always remember that you 

 to go into hem-onc, so soak up the day-to-day learning as it comes.

It can be challenging to stay on top of the list in the ruckus of a consult service, much less finding the time to do background reading. I found the reading that 

“sticks” the most is directly related to the patients I am actively following. UpToDate is an invaluable resource on the fly, and even a quick 5-minute refresher before heading into the patient room or presenting to the attending adds up. If you have more time, PubMed review articles relevant to the disease topic, How I Treat articles for hematology, and the “Principles of” sections in the back of each National Comprehensive Cancer Network guideline document for oncology are also great places to start. For more rigorous textbook-style review, check out the American Society of Clinical Oncology Self-Evaluation Program and the American Society of Hematology Self-Assessment Program.

In the vast majority of fellowships, first-year fellows are expected to respond to new acute leukemia consults from the emergency department. Hematologic emergencies aside, you will also be asked to review numerous peripheral smears during hematology rotations. A secret gem, and one of the most enjoyable rotations of first year, was my hemepath elective. Working with a seasoned hematopathologist, I had the opportunity to enhance my peripheral blood smear review skills, and I even learned to interpret flow cytometry plots and bone marrow biopsies.

Despite having seasoned pathologists ready to help with interpreting diagnostic studies at most academic institutions, there is still nothing quite like coming face to face with pathognomonic cells under the microscope—especially at 3 am. In fact, hematology-oncology fellows are expected to interpret their own bone marrow biopsies in many countries outside of the United States.

Most fellowships now allot up to 18 months for research. Hematology-oncology research spans the spectrum from basic, clinical, and translational to health services and global health. Even if your career plan does not include academia, fellowship still 

provides plenty of protected time to explore a subfield of hematology-oncology that might be of interest and dovetail with your future career.

The clinical demands are high during the first year but keep research plans in the back of your mind. Beginning in January, start reaching out to potential research mentors; by that point of your first year, you will have a good handle on how the hospital operates and can start laying the groundwork for research. Then hit the ground running once protected research time starts either later in the first year or second year.

Always remember to stay flexible with your plans, though. Highly anticipated projects may not ultimately get funded or an exciting data analysis may turn out to be a flop. Projects come and go, so find trusted mentors who will guide you through the ups and downs.

Like so many have said before about medical training: The days are long but the years are short. Don’t lose track of life outside the hospital—your family, friends, and hobbies are important. We live in an era of constant access to the electronic medical record on our portable devices and computers, but make a conscious effort to not log on during your off days. At this final phase of medical training, it is a good time to reflect upon the long journey up to this point (approximately 11 years not including gap years) and have a bit of “protected time” to determine your career goals before setting out into practice.

Congratulations! According to the National Resident Matching Program, hematology-oncology fellowship programs offered just 615 positions nationwide during the last cycle. We are joining a cadre of subspecialists tasked with providing cutting-edge and rapidly evolving care to an aging population with increasing hematology-oncology diagnoses by the year. Getting to this phase of training is an achievement but also an immense privilege and responsibility. In the words of the immortal Steve Jobs, “Stay hungry, stay foolish.” Read up on patients, tackle those smears, keep calm, and hem-onc on.

5 Tips for a First-Year Fellow

After 15 years as a surgeon, Jeffrey M. Farma, MD, says he's still learning how to treat patients with cancer. 

As fellowship director of the Complex General Surgical Oncology (CGSO) Fellowship at Fox Chase Cancer Center, he expects his trainees to be prepared to do the same.

Farma earned his MD at Temple University’s Lewis Katz School of Medicine in Philadelphia, Pennsylvania, before completing surgical fellowships at the National Cancer Institute (NCI) in Bethesda, Maryland, and Moffitt Cancer Center in Tampa, Florida. He returned to his native Philadelphia in 2009 when he joined the Department of Surgical Oncology at Fox Chase. He also serves as chair of the program directors’ committee for the Society of Surgical Oncology.

A surgical fellowship is different from other oncology programs because instead of focusing on a single tumor type, the surgeon has to learn about all of them. Farma said that an oncological surgeon may eventually specialize in a particular disease site, but surgeons must come out of fellowship with a broad knowledge of many types of disease and many areas of the body.

“My job is to make them into cancer doctors, not just surgeons and not just oncologists, but to really have a full understanding of treating all aspects of the patient with cancer,” he said. “That’s my ultimate goal.”

What was the most important thing you learned during your fellowships?

Working at the NCI is just, in itself, an amazing experience, especially being exposed to people like Steven A. Rosenberg, MD, PhD, chief of surgery at the NCI [Rosenberg is a 2013 Giants of Cancer Care

 award winner], and researchers who have a pure perspective on cancer. I think the most rewarding part of my experience at the NCI, was that patients were traveling in from all over the world to be treated at the National Institutes of Health.

There had been a lot of times I [had] been told that a patient had no other options—that there was nothing else we could do. Sometimes that was the case, but at the NCI, I learned there was always something else. There’s always another option. There’s always something positive or something on the horizon or some ability to give hope to patients.

At Moffitt Cancer Center, I was exposed to talented academic surgical oncologists who were able to 

perform really important research, be involved in national guidelines and cooperative group trials, and run clinical trials, while also working as high-volume cancer surgeons. It really taught me, both in and out of the operating room, how to best treat patients with cancer, how to evaluate them, and how to follow them afterward. I learned how to not only be a part of a multidisciplinary team, but to also lead multidisciplinary teams, which I currently do.

What do you tell your trainees about managing their money?

I always advise the residents I work with, number one, you have to have the long-game strategy in play. I trained for 9 years after medical school to do what I do. If you were an economist or a financial planner, that’s not the best strategy. That puts you 9 years behind a lot of your peers who went into other industries. But more important, you have to do something you’re exceedingly passionate about. We have jobs that are time demanding and emotionally demanding, so you have to love what you do.

That said, I did finish my fellowship in debt and I’m still paying off my medical school loans. You do have to consider all these things. Some fellowships might allow you to do some extra work on the side, which I did during my fellowship to make a little extra money. I definitely wouldn’t choose a fellowship because of that, but I think that there are a lot of fellowships that have some leniency to allow for moonlighting. And then, eventually, you start working and things gradually improve.

Are there skills that fellows, especially surgical fellows, 

They have to learn when to operate, but more importantly, when not to operate. That’s a very, very important skill to learn. They need to learn how to effectively stage and initially evaluate patients for cancer. Then after surgery, they have to learn how to follow the patients. 

We generally see all the patients for surveillance for years to come.

They need to know how run and manage a clinic and how to run teams–leadership, surveillance, and survivorship. They also need to know the palliative side of surgical oncology, how best to lead and manage difficult conversations. These are all aspects that I think are really important in the 2-year fellowship.

It’s definitely not just cutting. Our surgical oncology fellowship is exceedingly competitive. A large majority of the applicants now have multiple research publications. They’ve worked in the clinic and taken dedicated time off to do research. Many of them have advanced degrees, including MBAs, MPHs, and PhDs. We really are getting the cream of the crop. So another thing I say to all the applicants is, I can make them slightly better surgeons and show them some tips and tricks, but we need to concentrate on cancer care. Being a surgeon is a continuum. You don’t just finish. I’m still learning and becoming a better surgeon every day. But we are, more importantly, teaching them how to think about cancer, which is the most important thing for me.

What do you enjoy about working with fellows?

I’m heavily involved in education be it through medical students, physician assistant students, residents, and our fellows, who are really near and dear to my heart and training. There is nothing that makes my job more rewarding than seeing the success of others, seeing them progress in their career, and doing great things. Fox Chase was among the first surgical oncology programs in the country, and now we have networks of physicians who are practicing throughout the world who have been trained here and improving cancer care for patients in their communities. That’s really rewarding.

Surgical Oncology Is More Than Just Surgery

I am delighted to serve as the editor of this outstanding publication that connects trainees to hot topics in oncology. Over the years, Oncology Fellows has covered many topics including emerging areas of clinical care, the business of medicine, and wellness and burnout, among others. As your editor, I will write about emerging subjects that pertain to all of us in the hematology-oncology community. Thus, I want to take this opportunity to discuss this moment in time. Take a minute and think about what you were doing, your concerns, your worries, this time last year. We had not yet experienced the coronavirus disease 2019 pandemic and the subsequent economic challenges it created, the resurgence of active civil unrest, or questions about the proper role of policing in our society. Any one of those events would cause massive social upheaval, and we’ve seen all of them in 2020. Plus, we’re heading into a very important election period in November, and I haven’t even mentioned the murder hornets.

How does this apply to you as a fellow? As a physician, you are entrusted with the physical and, in part, emotional welfare of a community. You serve as a role model, and thus your words, your gestures, even your facial expressions hold great import.

Trainees are immersed in the business of being a trainee. This is incredibly demanding and emotionally draining at times. When I was a trainee, I did not become a part of the larger world outside of the hospital and the fellowship program. I was immersed in the “shelter” of medicine.

However, and I say this with some amount of shame, I recognized that health care inequity and racial injustice existed at that time, but that I did not name it or call it out as wrong. At this critical juncture, as I write this in the summer of 2020, none of us have the space or time to remain sheltered. All of us must play a role in how we move forward as a society. All of us must participate irrespective of party affiliation.

What does this mean? I urge you to become an active participant in this larger world. By virtue of being physicians, you are all community leaders. A great leader once noted that the distinction between a good leader and a great one is the ability to know when compromise is inappropriate. It is normal to live with fear, but we need to examine our fears a bit further. Do we fear that someone who is different threatens us? Are we afraid of speaking up? Do we fear self- examination or feedback? If we live in fear, we are our own prison guards. I urge you to not set limits on your involvement in this world. I also urge you to remain an advocate, not only for yourself, but for all.

Thomas Edison once said, “Our greatest weakness lies in giving up.” Sometimes swimming against the current is hard, and compassion fatigue is easy. This is not the time to give up. This is the time to not only aggressively pursue your education, a more traditional training focus, but also to recognize your own unconscious bias, speak up when you see injustice, encourage people to wear a mask, and register and take the time to vote. It is a seminal moment for your own personal and professional development. Avail yourself of every growth opportunity, find your voice, and define your sustainable path.

Now Is the Time for Action in Oncology 

Mentoring is critically important to the development of young academic hematology/oncology trainees. Key elements of an optimal mentor-protégé relationship include regular meetings, setting expectations, making time to listen, assessing progress, and promoting protégés.

Chief among those elements is effective 2-way communication, which, when working well, is a major facilitator of mentor and protégé success. When working poorly, such communication is a major barrier to a successful relationship.

The novel coronavirus disease 2019 (COVID-19) pandemic affected several aspects of medical practice, including continuing medical education and trainee mentoring. In-person mentoring was possible but infeasible because of social distancing and could not address the challenges of a rapidly evolving educational and practice climate. Consequently, many mentorprotégé relationships were no longer operating under their optimal context, especially with respect to regular meetings and effective communication. 

To address these challenges, a group of trainees and fellows created the Hematology Oncology COllaborative VIDeoconferencing (COVID) Learning Initiative. The project is a trainee driver, multi-institutional online learning platform designed to bring together junior physicians and expert faculty from around the country to collaborate on weekly COVID-19 teaching and to facilitate ongoing remote mentorprotégé relationships centered around program leadership and/or educational presentations.

In just 6 weeks, the initiative brought together a leadership team of 17 protégés and 11 educational mentors from 13 institutions, as well as 11 presenting protégés and 26 topic advisory mentors from 17 institutions. Together, they produced 5 multidisciplinary, online COVID-19 conferences that attracted an average of 100 providers.

Beyond learning about treating patients with cancer during the COVID-19 pandemic, participating in 

this multi-institutional, remote learning network has been an opportunity for us to learn about the challenges, and benefits, of virtual mentoring. In our experience, we found that the following 3 components are necessary to make virtual mentoring and collaboration a success: "

We believe that the model developed and used by the COVID Learning Initiative can be expanded to provide other learning, research, and clinical opportunities for fellows in training.”

As is true in any mentoring relationship, setting expectations is essential to keep remote mentoring on track. It is important to develop a project timeline with specified deadlines to clearly define roles and responsibilities for both mentors and protégés. Because our project included multiple participants at all levels collaborating on content, as well as multiple groups creating content at varying points in time, we opted to create a document of expectations to share with all participants. This provided an outline to ensure consistency in the content created and also developed a baseline mutual understanding of individual roles and collective group objectives.

There Is No Such Thing as Overcommunication

When members of a multi-institutional group like ours are unable to meet face-to-face, creating clear pathways for rapid and regular communication is essential. Nothing stalls a project’s momentum like an unanswered email, so develop the habit of staying on top of your inbox and replying immediately, even if it is just to confirm email receipt and let the sender know when he or she can expect a detailed reply.

Keep in mind which of the many media available is appropriate for each type of communication. Web conferencing platforms such as Zoom are great for facilitating group discussions, but they should be used sparingly since they take up significant time and can be 

difficult to schedule. Furthermore, the best meetings on these platforms require a leader to guide the conversation, as the lack of in-person social cues can lead to long silences or strings of interruption that stall idea sharing.

Use email for answers to specific questions from specific individuals. Keep in mind that emails with larger questions for larger groups can lead to multiple responses, crowded inboxes, and a lack of clear consensus. When creating online documents or presentations, consider incorporating shared-document editing platforms such as Google Drive rather than sending drafts back and forth.

Create A Level Playing Field to Share Perspectives

For true collaboration to occur, all contributors need to feel that their perspective is essential, valued, and heard. Because all providers were effectively novices in treating COVID-19, there was an inherent appreciation within our group for each individual’s experience and ideas, regardless of training level. That served to improve collaboration and led to the creation of educational content to meet the needs of a range of learners. It also provided an opportunity for trainees to emerge as leaders, showcasing or developing their talents for mentoring each other and serving in an organizational capacity.

Outside of COVID-19, creating this level playing field requires more deliberate action, particularly on the part of mentors and/or more senior group members. This is particularly true in virtual communication—silence can be interpreted negatively. Senior participants should make an effort to affirm the ideas of others, verbally or in writing, and to solicit input from group members who may be hesitant to join the conversation.

Our experience demonstrates that it is possible to bring together trainees and faculty across multiple institutions to identify learning priorities, develop educational content, and disseminate best practices rapidly in response to the COVID-19 pandemic. We have been able to leverage our collaborating institutions to provide robust technological support for our platform, tap into academic enthusiasm within the hematology-oncology community, and harness the visionary leadership that exists within the trainee community to make this platform a success. In doing 

so, we also learned important lessons about fostering successful interactions on virtual platforms to facilitate mentorship and collaboration.

Although we were initially hopeful that the pandemic would be under control within months, the recent resurgence of cases across the country and the attendant precautionary measures highlight the continued need for remote mentoring solutions. We believe that the model developed and used by the COVID Learning Initiative can be expanded to provide other learning, research, and clinical opportunities for fellows in training. The Accreditation Council for Graduate Medical Education recently clarified that remote mentoring of trainees during their fellowship is acceptable toward meeting its subspecialty credentialing and board certification requirements, making opportunities previously limited by the need for in-person preceptorship from a board-certified attending physician more readily accessible to all.

Our educational platform provides an opportunity for fellows to develop skills for using technology to learn, teach, and even mentor each other, which may help augment classic faculty-trainee and mentor-protégé relationships by providing a less intimidating arena for trainees to explore their own ideas and questions.

We hope that the lessons we learned in developing this mentorship network can be applied to many other areas, allowing trainees, seasoned providers, and investigators to build virtual collaborations to advance both fellows training and the care of patients with cancer.
