In today’s episode, we had the pleasure of speaking with Heinz-Josef Lenz, MD, FACP, about the prognostic and predictive value of HER2 gene expression for patients with 

 wild-type metastatic colorectal cancer (mCRC). Lenz is a professor in the Department of Medicine and the Department of Preventive Medicine in the Division of Oncology at the University of Southern California (USC) Keck School of Medicine in Los Angeles. He also serves as the associate director of Clinical Research, chair of the Gastrointestinal Oncology Program, and co-director of the Colorectal Center at the USC Norris Comprehensive Cancer Center.

According to data from a retrospective analysis of the patient population from the phase 3 CALGB/SWOG 80405 trial (NCT00265850), patients with mCRC with high HER2 expression experienced progression-free survival (PFS) and overall survival (OS) benefit regardless of treatment with bevacizumab (Avastin) or cetuximab (Erbitux). Notably, the association between HER2 expression and survival outcomes was nonlinear, with increases in OS, PFS, and overall response rate plateauing after reaching the median value of HER2 expression. Furthermore, the study found that among patients with high HER2 expression, treatment with cetuximab plus chemotherapy was associated with improved OS vs bevacizumab plus chemotherapy. Conversely, patients with low HER2 expression achieved superior PFS and OS with the bevacizumab regimen vs the cetuximab regimen.

In our exclusive interview, Lenz discussed critical gaps in knowledge regarding the role of HER2 expression in mCRC, expanded on the link between HER2 expression and responses with bevacizumab- vs cetuximab-based regimens, and emphasized the potential utility of HER2 expression to guide treatment decision-making in this setting.

Lenz Discusses the Prognostic and Predictive Value of HER2 Gene Expression in KRAS Wild-Type mCRC

In today’s episode, we had the pleasure of speaking with Andrzej Jakubowiak, MD, PhD, a professor of medicine and director of the Myeloma Program at the University of Chicago in Illinois. Dr Jakubowiak joined us to discuss interim findings from the phase 3 ATLAS trial (NCT02659293), which investigated lenalidomide (Revlimid) plus carfilzomib (Kyprolis) and dexamethasone (KRd) vs lenalidomide maintenance in patients with newly diagnosed multiple myeloma who had previously received induction therapy and autologous stem cell transplant.

At a median follow-up of 33.8 months, patients who received KRd had a median progression-free survival (PFS) of 59.1 months vs 41.4 months in those who received lenalidomide alone. This translated to a 49% reduction in the risk of progression or death in the KRd arm. Additionally, the ATLAS investigators found a correlation between high minimal residual disease negativity and improved PFS in the KRd arm.

In today’s episode, we had the pleasure of speaking with Sameek Roychowdhury, MD, PhD, a medical oncologist and an assistant professor in the Department of Internal Medicine and the Department of Pharmacology at The Ohio State University.

Therapeutic options are sparse for patients with advanced cholangiocarcinoma following progression on first-line gemcitabine-based therapy. Moreover, 10% to 16% of patients with intrahepatic cholangiocarcinoma harbor 

 fusions or rearrangements. The FDA has issued accelerated approvals for FGFR inhibitors such as erdafitinib (Balversa) and pemigatinib (Pemazyre) for use in this population. In April 2021, the agency granted a breakthrough therapy designation to a third FGFR inhibitor, futibatinib.

In May 2021, the FDA gave the green light to infigratinib, a selective, ATP-competitive inhibitor of FGFR, for the treatment of patients with previously treated locally advanced or metastatic cholangiocarcinoma harboring a 

 fusion or rearrangement. The regulatory decision is based on data from a pivotal phase 2 trial, which showed that a median follow-up of 10.6 months, the blinded independent central review–assessed objective response rate with the agent was 23.1% (95% CI, 15.6%-32.2%); this included 1 complete response (1%) and 24 partial responses (22%).

Today, we had the pleasure of speaking with Toni K. Choueiri, MD, director of the Lank Center for Genitourinary Oncology, director of the Kidney Cancer Center, and senior physician at Dana-Farber Cancer Institute, as well as Jerome and Nancy Kohlberg chair and professor of medicine at Harvard Medical School, and Robert J. Motzer, MD, the Kidney Cancer section head in the genitourinary oncology service and Jack and Dorothy Byrne chair in Clinical Oncology at Memorial Sloan Kettering Cancer Center.

Both Choueiri and Motzer were authors on the CheckMate-9ER study, results of which were recently published in the 

and were the basis for the January 2021 approval of nivolumab (Opdivo) and cabozantinib (Cabometyx) as a frontline treatment for patients with advanced renal cell carcinoma (RCC).

In the phase 3 CheckMate-9ER trial, investigators set out to examine the safety and efficacy of nivolumab/cabozantinib vs sunitinib (Sutent) as a frontline treatment in patients with advanced RCC with clear-cell histologic features. 

Data showed that the combination of nivolumab and cabozantinib showcased significant benefit in terms of progression-free survival, overall survival, and responses vs sunitinib in the frontline treatment of patients with advanced RCC.
