Tony SK Mok, MD, chairman, Department of Clinical Oncology, Li Shu Fan Professor of Clinical Oncology, fellow of ASCO (FASCO), member, ASCO Board of Director, member, The Hong Kong Academy of Sciences, discusses the rationale for the phase 1 U31402-A-U102 trial (NCT03260491) in metastatic or unresectable non–small cell lung cancer (NSCLC).

U31402-A-U102 examined the safety and antitumor efficacy of patritumab deruxtecan (U3-1402) in a 2-part trial consisting of dose escalation and dose expansion in patients with NSCLC. The trial found resulted in a recommended phase 2 dose 5.6 mg/kg, Mok says.

Moreover, patritumab deruxtecan elicited an overall response rate of 39% and a median progression-free survival of 8.2 months across all doses, Mok continues. In this heavily pretreated patient population, the median number of prior systemic therapies was 4, including previous TKI and chemotherapy, Mok adds. Despite prior treatment, the patients achieved impressive outcomes, setting the stage for future studies, Mok concludes.

It was an honor to become a Giant of Cancer Care®, according to Mok. Having been trained as a medical professional in Canada and having moved back to Hong Kong, China over the course of 25 years, Mok was able to establish a link between Asia, the United States, and Europe. This kind of link allows doctors from around the world to work together and collaborate to ultimately improve the quality of cancer research in the Asian-Pacific region and hopefully benefit cancer care on a global scale, Mok says.

This award is a recognition of this effort to connect researchers, says Mok. However, Mok explains that this is not an effort that is being made by him alone; rather, this a global effort is being made to link Asia with the rest of the world in the fight to improve care for patients with cancer.

In the ALEX trial, patients with newly diagnosed 

-positive NSCLC were randomized to receive alectinib (Alecensa) versus crizotinib (Xalkori). Data presented at the 

 showed that the median overall survival had yet to be reached in the alectinib arm versus the crizotinib arm. However, the 4-year OS rate was 64.5% (95% CI, 55.6-73.4) in the alectinib arm versus 52.2% (95% CI, 42.6-64.8) in the crizotinib arm. In terms of safety, fewer patients on alectinib experienced grade ≥3 adverse events versus those on crizotinib, at 48.7% and 55.0%, respectively.

In 2017, the FDA approved alectinib for the frontline treatment of patients with 

-positive NSCLC based on favorable progression-free survival data from the ALEX trial. As such, alectinib was adopted as the frontline standard of care for this patient population. Data from the OS analysis confirm that alectinib should remain the frontline standard, concludes Mok.

Tony S.K. Mok, BMSc, MD, FRCPC, professor, Department of Clinical Oncology, Chinese University of Hong Kong, Prince of Wales Hospital Hong Kong, China, discusses how to improve the strategy of developing treatments for lung cancer.

Mok says in order to move forward, multiple parties such as pharmaceutical industries, academic institutions, and the FDA have to make changes.

As an example, Mok says, the FDA should look into how it registers drugs because the current practice of demanding a companion diagnostic may actually be slowing down the development of the drug.

Mok also suggests making changes to the pharmaceutical industry by having external tribunals advise pharmaceutical companies on which drugs to develop rather than wasting money by trying to develop many drugs. 

Dr. Mok Discusses Developing Lung Cancer Treatments

Tony S.K. Mok, BMSc, MD, FRCPC, Professor, Department of Clinical Oncology, Chinese University of Hong Kong, Prince of Wales Hospital Hong Kong, China, discusses different plasma testing approaches for patients with lung cancer.

Mok says that a physician has the option to conduct a plasma test when the diagnosis is being made. By the time the lung cancer diagnosis is confirmed, a physician will have plasma testing results available. If the test is positive, treatment can begin, but if it is negative, a physician can subject the tumor for testing.

If good tissue cannot be obtained for testing, a physician can use plasma as a testing method but should be aware that sensitivity is only 76%.
