There are several emerging cellular therapies in the multiple myeloma space, which are expected to receive regulatory approval within the next year. The first is idecabtagene vicleucel (ide-cel; bb2121), which targets BCMA, Nagle says.

Next, there is JNJ-4528, an investigational, BCMA-directed CAR T-cell product which is currently being evaluated in the CARTITUDE-1 study for the treatment of patients with relapsed/refractory multiple myeloma. This product has shown very encouraging results thus far, Nagle says.

Notably, despite the host of BCMA-targeted agents that are currently under exploration, there is an increasing interest in discovering new targets, especially for patients who relapse after BCMA-directed therapies, Nagle concludes.

Historically, daratumumab had a very long administration time, says Nagle. As patients tolerate the infusions, the administration time is shortened.

In May 2020, the FDA approved a subcutaneous formulation of daratumumab and hyaluronidase-fihj (Darzalex Faspro) for use in adult patients with multiple myeloma. 

Although patients are monitored for a period of time after receiving the subcutaneous regimen, the transition significantly shortens the administration time of daratumumab, which allows the agent to be more accessible to patients, concludes Nagle.

Developing an optimal sequencing strategy can be difficult in multiple myeloma considering all the available treatment options, Nagle says.

Currently, the question of whether certain treatments should be utilized up front or reserved for progression remains unanswered, Nagle says. The answer to this question may inform treatment sequencing.

Additionally, some patients who do not respond to novel agents respond to cytotoxic chemotherapy, explains Nagle. As such, identifying these patients earlier will aid in optimizing their treatment course. 

Although having a multitude of treatment options is beneficial, utilizing them in the appropriate sequence may remain a significant challenge, concludes Nagle.
