Rodolfo Bordoni, MD, medical oncologist, director of clinical research, leader, Phase I Clinical Trials Program, Georgia Cancer Specialists, discusses the utility of ​atezolizumab (Tecentriq) in the adjuvant setting in non–small cell lung cancer (NSCLC).

Atezolizumab demonstrated efficacy when used as adjuvant therapy for patients with NSCLC who underwent resection and platinum-based chemotherapy, Bordoni says. However, this is only the case when patients have PD-L1–positive disease on at least 1% of tumor cells; PD-L1–negative patients did not respond to adjuvant atezolizumab, Bordoni explains. 

Moreover, on October 15, 2021, the FDA approved adjuvant atezolizumab for patients with stage II to IIIA NSCLC following resection and platinum-based chemotherapy whose tumors have PD-L1 expression on at least 1% of tumors cells, as determined by an FDA-approved test. 

Moreover, the role of adjuvant therapy is an area that needs additional research, Bordoni continues. Several clinical trials are being conducted evaluating immunotherapy alone or in combination regimens, and these data are highly anticipated, Bordoni concludes.

Rodolfo Bordoni, MD, medical oncologist, director of research, Georgia Cancer Specialists, discusses the utility of ​adjuvant osimertinib (Tagrisso) in lung cancer.

Osimertinib has impacted the standard of care in the adjuvant setting following it’s FDA approval for use as adjuvant treatment for patients with non–small cell lung cancer (NSCLC) whose tumors express 

 exon 19 deletions or exon 21 L858R mutations. The approval was based on findings from the phase 3 ADAURA trial (NCT02511106), in which patients with stage IB to IIIA NSCLC had improved disease-free survival with adjuvant osimertinib vs placebo, Bordoni explains. 

It is important to consider how to improve quality of life for patients by understanding when to offer patients adjuvant therapy with chemotherapy or immunotherapy, Bordoni explains. By gaining more knowledge on patient tumors through ongoing studies, it will be possible to avoid causing unnecessary adverse effects in patients, Bordoni adds.

Historically, the benefit of adjuvant therapy has been approximately 10%; however, it is important to continue to identify which patients are best suited for adjuvant therapy to increase this rate, Bordoni concludes.

Rodolfo Bordoni, MD, a medical oncologist, and director of research at Georgia Cancer Specialists, discusses the importance of the phase 1/2 LIBRETTO-001 trial (NCT03157128) with selpercatinib (Retevmo) in patients with 

 fusion–positive non–small cell lung cancer (NSCLC).

Findings from the trial were impressive for 2 reasons, according to Bardoni. First, the trial marks the first time in which a specific inhibitor of 

 fusion, selpercatinib, was utilized for patients with lung cancer, thyroid cancer, and other conditions that may carry a 

 aberration, Bardoni says. Previous targeted therapies that were being used in this population were not specific inhibitors, which led to a limited efficacy. Additionally, these older agents were limited in both the patients who responded to treatment, as well as in those who experienced a prolonged response and clinical benefit, he explains. 

Secondly, the previous targeted agents were multitarget, and targeted different angiogenic gene products. Due to this, the agents caused a significant number of adverse effects, Bardoni concludes.
