Sunitinib was shown in the S-TRAC trial to modestly improve disease-free survival for patients with completely resected RCC, explains Lara. Sunitinib as an adjuvant therapy has considerable toxicities that need to be managed. 

According to Lara, even though sunitinib did see an increase in disease-free survival it had no clear benefit in terms of overall survival.

Preliminary results from the phase II ECHO-202/KEYNOTE-037 trial showed encouraging response rates and safety signals with epacadostat plus pembrolizumab (Keytruda) in patients with metastatic RCC.

Lara says the advantages of a drug like epacadostat is that it is oral, so it provides an extra level of convenience, should it become a standard of care in the future.

Primo N. Lara, MD, associate director for translational research, UC Davis Comprehensive Cancer Center, discusses epacadostat plus pembrolizumab (Keytruda) in patients with metastatic renal cell carcinoma (RCC).

The phase Ib/II trial showed encouraging response rates and safety signals for this combination, particularly with patients who had minimally pretreated disease.

The confirmed response rate was 47%. In this study, only 17% of patients experienced grade 3 or higher toxicity, which is what is expected with single agent checkpoint inhibitor therapy.
