Multiple targeted therapies and combinations of targeted therapies are available for the treatment of patients with 

-mutated multiple myeloma, and each targeted therapy is associated with different AEs, Yorio says. For example, a higher incidence of pyrexia is associated with the combination of dabrafenib (Tafinlar) and trametinib (Mekinist), Yorio explains.

Although the combination of encorafenib (Braftovi) and binimetinib (Mektovi) produces lower rates of pyrexia, the 2 agents are linked with increased eye toxicity, Yorio continues. The combination of vemurafenib (Zelboraf) and cobimetinib (Cotellic) is associated with increased rates of diarrhea and other gastrointestinal AEs, Yorio says. Understanding the AEs associated with each regimen can help inform treatment decisions for individual patients with

The various combination therapies available for the patients with melanoma harboring 

 mutations include vemurafenib (Zelboraf) plus cobimetinib (Cotellic), dabrafenib (Tafinlar) plus trametinib (Mekinist), and binimetinib (Mektovi) plus encorafenib (Braftovi), Yorio explains, adding that they have produced high initial response rates in this patient population.

-mutated melanoma can experience tumor shrinkage when treated with these targeted therapy combinations, Yorio continues. The up-front use of these agents can produce a rapid response in patients with a high tumor burden who are symptomatic from metastatic disease, Yorio concludes.

The identification of rare molecular aberrations in metastatic lung cancer has increased the utility of up-front genetic testing, explains Yorio. Moreover, targeted therapy may be an option for patients with specific mutations.

Clinical trials may be an alternative for patients with an alteration for which there is no available treatment strategy; however, it can be difficult to enroll patients with rare alterations like 

 exon 14 skipping mutations in large clinical studies, says Yorio.

Pooling patients with a rare abnormality is important to demonstrate the efficacy of an investigational drug and develop more efficient models to treat patients with potentially actionable mutations, concludes Yorio.

Jeff Yorio, MD, medical oncologist, Texas Oncology, discusses the search for biomarkers and using next-generation sequencing (NGS) in metastatic lung cancer.

For patients diagnosed with metastatic lung cancer, it is now standard of care to perform NGS with tissue, explains Yorio. Biomarkers in lung cancer have expanded beyond 

 exon 14 skipping, and more with research continuing to search for other subgroups of patients with distinct biomarkers.

NGS assists in creating personalized treatment plans for patients, especially as new therapies are developed to treat patients with certain biomarkers, says Yorio. Even though all the patients are diagnosed with metastatic lung cancer, they may require different paths of treatment depending on any biomarkers that are present. Additionally, NGS panels are more effective than single mutational tests because it searches for multiple biomarkers, allowing for less tissue to be used, concludes Yorio.

Patients involved in the study were heavily pretreated with traditional treatments for metastatic lung cancer, including chemotherapy and immunotherapy. The partial response rate to ado-trastuzumab emtansine in this study was 44% and stable disease was observed in 39% of patients.

Ado-trastuzumab emtansine is not yet approved for use in lung cancer; however, the National Comprehensive Cancer Network is recommending the drug for 

-mutant patients with metastatic lung cancer.
