The ZUMA-7 study is evaluating the efficacy of the chimeric antigen receptor (CAR) T-cell therapy axicabtagene ciloleucel (axi-cel; Yescarta) compared to standard of care therapy in patients with relapsed/refractory DLBCL. In this phase III randomized, open-label, multicenter study, patients will be randomized to receive axi-cel or standard of care second-line therapy following treatment with first-line rituximab (Rituxan) and anthracycline-based chemotherapy.

Winter says that immunotherapy may be the best strategy for patients with relapsed/refractory DLBCL. This is very exciting for these patients, Winter explains, because there are not many therapeutic choices for this population.

The intravenous pan-class I PI3K inhibitor copanlisib has shown predominant activity against the PI3K-alpha and -delta isoforms in follicular lymphoma.

Winter says that the category of PI3K inhibitors continues to grow in this disease, and this class of agents will positively add to the armamentarium.

Checkpoint inhibition might have some toxicity issues, explains Winter. However, the CheckMate-205 trial, presented by Michelle A. Fanale, MD, at the 14th International Conference on Malignant Lymphoma in Lugano, Switzerland, indicated that there has been little to no serious toxicity in the patients who received nivolumab (Opdivo).

Winter is hopeful that these agents for patients with Hodgkin lymphoma will have a low toxicity profile.

It is too early to determine benefit of CAR T-cell therapy in follicular lymphoma, says Winter, with responses ranging from 40% to 50%. Although, there may be promise in combining other agents or altering the technique.

A large benefit has been seen in other hematologic malignancies such as diffuse large B-cell lymphoma, states Winter.

Follicular lymphoma is a rapidly changing field, says Winter.

Although recent evidence has shown that most patients with follicular lymphoma will do very well on therapy, about 20% will progress within the first 2 years. There is no effective way to identify this population as of now, adds Winter.
