Patients with metastatic bladder cancer whose tumors did not respond to chemotherapy should consider using one of the checkpoint inhibitors, states Inman.

The other situation is for patients who are ineligible for cisplatin. According to Inman, if a patient cannot receive cisplatin, the best choice is to consider a checkpoint inhibitor.

There are not currently good biomarkers for immunotherapy, explains Inman. However, the molecular subtypes of bladder cancer can predict response to immunotherapy.

According to Inman, these biomarkers are more telling about the biology of the tumor, rather than the response to a specific treatment.

Although there have been many trials with chemotherapy combinations, there has not been much progress seen beyond gemcitabine plus cisplatin or with the combination of methotrexate, vinblastine, doxorubicin, and cisplatin (MVAC), says Inman.

Bladder cancer is being better understood with the utilization of genome sequencing and RNA sequencing, both of which have allowed for this disease to be categorized into several different molecular subtypes.

Tests to check for these subtypes will become important in the future, Inman says, as it will help guide treatment for patients.
